A run-flat is built to be driven on with no air in it, which is useful exactly once and misleading for ever afterwards. The reinforced sidewall that got you home is the part that takes the damage, and none of it shows from outside. The tire looks like a tire. The only honest answer comes from taking it off the wheel.

Why a run-flat needs the full inspection rather than a look
The base rules apply to every tire, from the US Tire Manufacturers Association: tread area only, damage no wider than a quarter of an inch, the tire off the wheel so the inner liner can be inspected, and a plug and a patch fitted together. Run-flats add two things on top.
The reinforcement. A run-flat carries a stiffened sidewall designed to hold the car up for a limited distance at a limited speed with no air in it. Go past either limit and the reinforcement is cooked while the tire still looks normal. So tell us how far you drove on it and roughly how fast. That answer changes the likely outcome more than anything else you can give us.
The manufacturer. Whether a specific run-flat may be repaired at all is a question for the company that made it, on top of the industry standard. We check both, and if the answer is no we say so rather than patching it and hoping.
Georgia's tire statute happens to describe the failure mode precisely. O.C.G.A. 40-8-74(e) requires all tires to "be free from any cuts, breaks, or snags on tread and sidewall deep enough to expose body cord" and to "be free from bumps, bulges, or separations". A run-flat driven beyond its rating is a separation waiting to be found, and it will have plenty of tread left while it is one.
The spare you probably do not have
Cars delivered on run-flats often ship without a spare wheel, because the tire was the spare. That turns a flat into a bigger event than it looks: no fifteen-minute swap in a parking space, and a straight choice between a tow and a technician who comes to the car.
It also sharpens the replacement decision. We fit run-flats where the size is available locally, and we will not put standard tires on a car built for run-flats unless you confirm you understand the vehicle may then have no spare of any kind. That is a real trade to make deliberately, not a default. Atlanta publishes three different answers on run-flats
This is the part you cannot get anywhere else. The Tire Rack recommended-installer directory for ZIP 30306 lists three mobile installers, and all three itemise a run-flat line. Read live on 2026-09-20:

Same city, same directory, same day, three different treatments of the same line. And all three of those cards price fitting. Not one of them prices a puncture repair of any kind, on a run-flat or on anything else.
Two of the three also publish a restriction on what they will touch at all, and these are the only published vehicle-fit restrictions we found for mobile fitting in Atlanta. Tire Rack Mobile Installation's Atlanta listing states, verbatim: "This installer does not install tires or wheels smaller than 13." Tirewire's states, verbatim: "Center Lock Wheels". Worth checking against your own car before you assume any of this is available to you.
One local operator does publish a repair scope, and it excludes the case people most often ask about. 3030 Roadside: "We DO NOT repair donut tires, temporary tires, or tires with sidewall damage."
Nothing in Georgia will tell you the tire is finished
Georgia sets a tread figure — 2/32 of an inch in all major grooves, for every vehicle, under 40-8-74(e)(1) — and schedules nobody to check it. Title 40 Chapter 8 contains five articles and none of them is a periodic safety inspection for a passenger car. The annual emissions test is an OBD scan plus a visual inspection of the catalytic converter.
On a run-flat that gap matters more than on an ordinary tire, because the whole design of a run-flat is to stop the car telling you anything is wrong. The tire does not go visibly soft. The TPMS warning is doing the work the sagging sidewall used to do, and if the sensor is dead you will not know. What happens at the visit
The wheel comes off, the tire comes off the rim, the inner liner and the sidewall reinforcement are inspected, and you get a straight answer. If it is repairable, we repair it, remount, rebalance that wheel, reset the TPMS where the vehicle supports it at the roadside, and set the pressures to the door placard. If it is not, the inspection is charged and the repair is not, and you decide what happens next.

A mobile puncture repair is $145 for the first tire and $50 for each additional tire in the same visit. That is the price, not an estimate and not a starting figure. It covers getting to you, the wheel off, the tire demounted and inspected from the inside, a plug and patch fitted the way the tire industry requires, remounting, rebalancing, a TPMS reset where the vehicle supports it, and the wheel back on. There is no separate call-out fee, and there is no run-flat surcharge. If the damage turns out not to be repairable, we tell you on the spot: the inspection is charged and the repair is not.

Questions people ask
- Can a run-flat be repaired at all? Sometimes. It has to pass the same US Tire Manufacturers Association test as any tire, and the tire's manufacturer has to permit repair. Both are checked with the tire off the wheel.
